CAREPASS by Alpro Health - Membership Terms & Key Information

BY CONTINUING, YOU AGREE TO THE CAREPASS TERMS & CONDITIONS AND PRIVACY POLICY.

CarePass is a preventive healthcare subscription programme designed to support access to selected healthcare services and promote early health awareness. It is not a medical insurance plan, hospitalisation plan, or emergency healthcare service, and does not provide indemnity or reimbursement for medical expenses.

1. Programme Owner & Structure

CarePass is powered by Alpro Health Sdn Bhd (Company Registration No.: 1593739M / 202401047894), backed by Alpro Group Malaysia. The Programme is supported by the ecosystem of Alpro Group Malaysia and its related entities, including participating healthcare providers, clinics, and service partners. Alpro Health Sdn Bhd acts as the Programme administrator and coordinator. Services may be delivered by employed or independent healthcare professionals, or third-party providers within the approved network. All fees stated are subject to applicable taxes where acquired by law. Alpro Health Sdn Bhd reserves the right to revise pricing for future renewals.

2. Definitions

“Member” refers to an individual who has successfully purchased, registered, and activated CarePass. “Subscription / Programme” refers to the CarePass membership. “Benefits” refer to healthcare services, access privileges, screenings, consultations, vouchers, discounts, or other entitlements provided under the Programme. “Participating Providers” refer to Alpro outlets, clinics, healthcare professionals, or approved partners delivering services under the Programme. “Validity Period” refers to twelve (12) months from the date of Activation unless otherwise specified. “Activation” refers to successful registration and commencement of membership/subscription usage and each card upon purchase requires activation within twelve (12) months before the card lapse.

3. Eligibility

The Programme is available to individuals residing in Malaysia. Members must be aged eighteen (18) years and above at the time of registration. Members are required to provide accurate, complete, and up-to-date personal information. Alpro Health Sdn Bhd reserves the right to verify identity prior to granting access to any Benefits.

4. Subscription, Activation & Validity

The Programme fee is RM99 nett per member per year unless otherwise stated. Membership commences upon Activation and remains valid for twelve (12) months. All included benefits may only be accessed after activation. Membership shall automatically expire upon completion of the Validity Period. Renewal is required to continue access to Benefits after expiry. No pro-rated extension, carry-forward, rollover, or reinstatement of expired Benefits shall be allowed. Upon successful subscription, a physical CarePass card with unique activation code will be provided to you, whether through physical outlets or Alpro One Click. Each card will have twelve (12) months of period for activation, the card is automatically terminated if no activation happens. No extension of validity shall be granted under any circumstances, including non-utilisation, scheduling conflicts, or personal reasons.

5. Scope of Benefits

Benefits are designed for preventive healthcare purposes and include: Complete 70-Marker Essential Blood Test Package worth RM85; Flu Vaccination (Up to 12-Month Protection) worth RM85; AI Fundus Eye Screening with Glaucoma Risk Review by Optometrist worth RM55; Ear & Audiometry Hearing Test with Audiologist Consultation worth RM55; Premium Eye Support with Powerlife Optiseen (30 Chewable Tablets) worth RM55; 45-Minute Neck & Back Relief Therapy with Physiotherapist worth RM110; Unlimited Online Doctor Consultation & Medication at RM29 per Visit worth RM135; Personal Accident Protection Coverage up to RM50,000 worth RM75; and Basic Dental Screening & Oral Health Advice (Coming Soon). Benefits are subject to availability at participating locations, appointment scheduling requirements with additional handling fees if schedule is out of available time slot, and professional assessment and suitability. Not all Benefits are available at all outlets or at all times. Benefits are strictly non-transferable, non-exchangeable and may only be used by the activated member.

6. Benefit Utilisation Conditions

Members must comply with all redemption procedures, including identity verification, appointment booking where required, and presentation of membership or digital confirmation. Certain services require advance booking. Failure to attend scheduled appointments may result in forfeiture of the Benefit. Benefits not utilised within the Validity Period shall lapse automatically. Additional charges may apply for services not explicitly included in the Programme, including but not limited to delivery fees, upgrades or extended services. Alpro Health Sdn Bhd reserves the right to modify, replace or discontinue any Benefit(s) within the Programme at its discretion.

7. Service Availability & Locations

Benefits may be redeemed at participating premises with availability and location listed in CarePass information page, under the subsection of “Redeem”. Selected services may be available at specialised centres, including Alpro Audiology, Alpro Physio and Alpro Optisaver. Service availability may vary by location, state, and operational capacity. Members are advised to confirm availability prior to visiting.

8. Appointment, Walk-In & Handling Fee

Services such as blood tests and vaccinations are provided based on scheduled appointments. Walk-in requests or services outside scheduled arrangements are subject to availability. A handling fee of RM5 may be imposed for walk-in services, rescheduling outside allocated slots, or special arrangements outside scheduled appointments.

9. Service Delivery & Professional Discretion

All healthcare-related services are subject to the professional judgment of the attending healthcare provider. Providers may decline or modify service approach with proper justification and/or clinical reasoning, and may refer Members to other healthcare facilities for further procedures or assessments. Screening results and consultations are provided for guidance only and do not constitute an absolute medical diagnosis. Alpro Health Sdn Bhd does not guarantee any specific health outcomes, results, or improvements from the use of the Programme or its Benefits. Members arriving more than fifteen (15) minutes late may be required to reschedule and the Benefit may be considered utilized.

10. Minor Ailment & Consultation Scope

Minor ailment consultations are limited to conditions suitable for community pharmacy or primary care management, run by Alpro e-Pharmacy and supported by registered medical practitioners. Providers may refer Members to external medical services if the condition exceeds the scope of the Programme. Any medications, products, or follow-up services may incur additional charges unless stated otherwise. No warranty is provided on effectiveness, and individual results may vary.

11. Personal Accident Coverage

Any personal accident coverage included under the Programme is provided by a licensed third-party insurer, AIG Malaysia Insurance Bhd. Such coverage is subject to insurer’s policy wording, terms and exclusions, claim procedures, and approval criteria. Alpro Health Sdn Bhd is not the insurer and shall not be liable for claim approvals or rejections, compensation amounts, or claims processing timelines. Alpro Health Sdn Bhd shall not be liable for services rendered by third-party providers, including clinics, practitioners, or partners. All inquiries and processing are to be forwarded directly to AIG Malaysia Insurance Bhd.

12. Fees, Payment & Refund Policy

Full payment is required at the point of purchase or subscription. CarePass is strictly non-refundable once full payment is complete. No refund shall be provided for unused benefits, change of mind, non-utilisation or dissatisfaction with service availability. CarePass subscription is not exchangeable for cash, points or non-monetary items at any time. Alpro Health Sdn Bhd reserves the right to cancel or suspend any subscription arising from failed, disputed, or fraudulent transactions.

13. Member Obligations

Members shall provide accurate and complete information, attend scheduled appointments, follow service instructions, and disclose relevant health conditions. Members shall not transfer or share membership, misuse Benefits, provide false or misleading information, or engage in fraudulent activity. Any suspected fraud, misuse, or unauthorized use may result in immediate suspension or termination without refund.

14. Data Protection & Communication

Members consent to the collection and use of personal data for Programme administration and service delivery. Health-related data may be collected where required for professional care. Members may receive communications including, but not limited to appointment reminders, service updates, and programme notifications. Alpro Health Sdn Bhd shall not be liable for any interruption, delay, or failure of digital platforms, including application or system downtime.

15. Disclaimer

CarePass is intended for preventive healthcare support only. It does not replace medical diagnosis, clinical treatment, emergency care or hospitalization services. Alpro Health Sdn Bhd shall not be responsible for any medical outcomes resulting from the use or non-use of Benefits.

16. Limitation of Liability

To the fullest extent permitted by law, Alpro Health Sdn Bhd shall not be liable for indirect or consequential losses, loss of opportunity and service dissatisfaction. Liability, if any, shall be limited to the amount paid for the Programme. Members agree to indemnify and hold harmless Alpro Health Sdn Bhd against any claims, damages, or liabilities arising from misuse of the Programme or breach of these Terms.

17. Variation of Terms

Alpro Health Sdn Bhd reserves the right to amend the programme structure, benefits, participating providers and operational terms at any time. Updates will be communicated via official channels where reasonably practicable.

18. Service Disruption & Force Majeure

Alpro Health Sdn Bhd shall not be liable for failure or delay in providing services due to events beyond reasonable control, including but not limited to public health emergencies, pandemics, government restrictions, supply shortages, system downtime or workforce constraints. In such circumstances, Alpro Health Sdn Bhd reserves the right to reschedule, modify or suspend affected services or Benefits.

19. Termination & Suspension

Membership may be suspended or terminated if misuse or abuse is detected, fraudulent activity is misuse or abuse is detected, fraudulent activity is suspected or terms are breached. No refund shall be provided upon termination due to breach.

20. Digital Access & Account Responsibility

Members are responsible for maintaining the confidentiality of their account credentials and registered mobile number. Alpro Health Sdn Bhd shall not be liable for unauthorized access or misuse arising from compromised devices or shared access. Any redemption made through the registered account shall be deemed valid. Alpro Health Sdn Bhd shall not be responsible for lost or damaged CarePass cards. Replacement may be subject to administrative fees.

21. Governing Law

These Terms shall be governed by the laws of Malaysia. Any disputes shall be subject to the jurisdiction of Malaysian courts.
